Partners Requiring Prequalification

We value the strong relationships we build with our subcontracting partners and welcome qualified firms to work with us. To help us maintain a safe, high-quality, and successful project environment, all subcontractors bidding and contracting to perform construction/trade work are asked to complete our prequalification process annually to remain eligible to bid on and contract for work with our company.

The following scopes of work DO NOT need to go through the prequalification process:

  • Vendors
  • Material Suppliers
  • Planning
  • Surveying
  • Testing
  • Adjusting
  • Balancing
  • Engineering Firms
  • Architecture, including Landscape
  • Environmental Science

Required Documentation

What do I need?

As part of the prequalification process, please complete the prequalification form below and provide the supporting documentation outlined below. To get started, at least one of the requested documents must be submitted with your prequalification form.

The remaining documentation can be uploaded with your application or provided separately by email. We appreciate your time and look forward to learning more about your company and the work you do.

Supporting Documents

Form W-9 (March 2024 version)

The official W-9 form is located on the IRS.gov website. Please provide your W-9, that includes your full legal name, signed and dated within the past year.

Sample Insurance Certificate

Please provide a current Certificate of Insurance (COI) along with all applicable endorsements.

The certificate should be current and not expired, as proof of insurance is required to complete the subcontractor prequalification process.

OSHA 300 and 300A

Please provide OSHA Forms 300 and 300A for the past three (3) years. If your company has 10 or fewer employees, please provide the number of employees and total hours worked for each year. For all company sizes, OSHA Form 300A is required if any recordable incidents occurred.

EMR Verification

Emergency Medical Responder (EMR) Verification for the past three 3 years

National Council on Compensation Insurance (NCCI) comparable form or letter from worker’s comp carrier.

Optional: Letter from Your Surety

A letter from your surety, dated within the last 30 days, confirming our single-project bonding capacity, aggregate bonding capacity, and current available bonding capacity.
